Our Goal at Integrative MD is to provide our patients with the highest level of personalized care. We are committed to helping you achieve optimal health and thank you for strictly following our practice policies.
Contact Information: To ensure timely and effective communication we require up-to-date patient contact information (name, address, phone number and email) on file. Patients are responsible for informing us of any changes in their contact information and preferred communication method in a timely manner.
Privacy: We strictly comply with all federal and state laws regarding storing, handling and release of protected health information. Our Notice of Privacy Practices is available at our office location and on our website.
Insurance: Patients are responsible for confirming their initial and continued eligibility to receive benefits with their insurance company. Patients are responsible for informing us of any changes in their insurance plan. Insurance claims denied due to inaccurate information provided to us may result in patient billed directly on a self-pay basis.
Copayments: Collecting copayments in full is required by the insurance. All copayments are due at the time of service.
Outstanding Balances: All outstanding balances must be paid in full before the next appointment is made.
Medical records can only be released with patient’s written authorization. A medical records release form is available upon request.
Communication: We communicate by telephone, email, and via secure messages on Patient Portal. All new patients are provided with free access to our patient portal where they can review their health information, personal data, lab results, and communicate with us in a secure way. We require an email address to send appointment reminders and notifications of important new information posted on the Patient Portal. Patients are responsible for reviewing all our communications carefully. We do not send private health information via regular email due to privacy concerns.
Cancellation / No-Show: We have a 24-hour cancellation policy. Patients cancelling less than 24 hours before the appointment and/or failing to show up for the appointment are billed a cancellation / now-show fee in effect at that time.
Late for Appointment: We are a busy practice and reserve the appropriate amount of time for each patient when scheduling an appointment. To ensure fairness to other patients we cannot guarantee that patients who arrive late for their appointment will be seen that day.
Prescription Refills: We require 72 hours to refill prescriptions from the time of request. To reduce refill time, we recommend that patients call their pharmacy directly. Depending on the prescription or the date of last visit, patients may be asked to make an appointment before the medication is refilled.
Mobile Phones: Conversations on mobile phones are not permitted anywhere in the office including the patient waiting area.
